Tax Law Changes are Driving Employer Healthcare Financing

The employee
benefit arena continues to be a challenge to businesses of all sizes.
As employers
struggle with funding these ever-increasing costs, it appears that for the
foreseeable future, the best way for employers to mitigate these rate increases
are to
become familiar
with and probably implement one or more of the following programs;
a premium only
plan(POP), a flexible spending account(FSA), a Health Reimbursement
Arrangement (HRA)
and or a Health Savings Account (HSA).
Let’s review these
briefly;
A Premium Only
Plan allows employers to withhold employee health insurance and other
benefits’ contributions on a “before tax” basis. By implementing this type of
plan, employees may elect to have their gross earnings reduced, but increase
their take home pay. The savings to the employee is the Social Security/Medicare
and Federal, State and local tax savings, amounting to 10-30%. The employer also
saves on Social Security/Medicare match, as well as wage based costs such as
worker’s compensation, general liability and others. This savings is at least
10% of the premiums the employees pay on pre-tax basis.
A Flexible
Spending Account (FSA) builds on the above plan. With an FSA, employees can
“set-aside” a portion of their pay on a pre-tax basis to pay for expected
medical expenses. The IRS guidelines for what is reimbursable under an FSA
greatly exceed coverage on most medical plans. Employees can save to pay
dental, vision, hearing, deductibles, co-payments, even over-the-counter (OTC)
medications. The employee and employer both benefit as above.
A Health
Reimbursement Arrangement (HRA) is a newer twist on an old tax law.
Basically, an HRA is a promise of an employer to reimburse certain medical
expenses to covered employees and or their dependents. This type of arrangement
allows an employer to participate in “partially self funding” insurance claims.
The results can be amazing. One of our clients implemented this type of plan and
has saved over $35,000 in premiums in seven months! This savings came from only
18 employees and their families. The flexibility and potential premium savings
make this a must see for employers, particularly with 10 or more employees.
The newest of these
changes is the Health Savings Account (HSA). The HSA was introduced
January 1, 2004 as a innovative approach to getting consumers to participate
more in utilizing cost-effective healthcare. The concept it to provide a high
deductible ($1,000 to $5,000) catastrophic health plan with a tax-deductible
Savings Account that can be used to pay expenses below the deductible. The
employee has the incentive to use this money wisely, as it is their own personal
account. In addition, whoever contributes to the Health Savings Account gets to
fully deduct the contribution, in some cases saving the participant $1,500 in
income taxes! These plans seem best for companies with fewer than 10 employees,
as the employers in this market will often contribute to the high deductible
insurance coverage, and let the employee save the money for the Health Savings
Account.
